Elevance Health is seeking a LTSS Service Coordinator Clinician in a field-based role, working under the supervision of an RN to coordinate care for members with chronic illnesses and disabilities. The position emphasizes field assessments, telephonic contact, and collaboration with the health care team to identify needs and develop care plans. The adecuado candidate will hold LPN/LVN, LSW, LCSW, or LMSW licensure (or RN exemption as allowed by state law), with at least 2 years in a similar role,
