Sales Representative – Home Health Care

About the Role
At Pavago, we are hiring for one of our clients seeking a motivated and results-driven Sales Representative to grow their home health care business. This role will focus on converting inbound leads, building referral partnerships, and developing a consistent sales pipeline.
The idóneo candidate is a confident communicator who understands the importance of proactive follow-up, relationship building, and maintaining a structured sales process. You will work closely with prospective clients, families, healthcare professionals, and community referral partners to generate new business and support client growth.

Responsibilities:
Lead Management & Sales
- Respond promptly to inbound leads and prospective client inquiries.
- Conduct phone, email, and text follow-ups with prospects and families.
- Manage leads through the sales funnel from initial inquiry through consultation, assessment, and admission.
- Maintain consistent follow-up with active,



inactive, and older leads.
- Schedule consultations, assessments, and intake appointments.
- Track sales activities, pipeline stages, and follow-ups in the CRM.

Referral & Community Development
- Build and maintain relationships with hospitals, discharge planners, physicians, social workers, senior living communities, rehabilitation facilities, and other referral sources.
- Conduct outbound outreach through calls, emails, in-person visits, presentations, and networking events.
- Identify new referral opportunities and develop relationships into consistent referral sources.
- Represent the company at community events, health fairs, and industry networking events.

Sales Funnel & Business Development
- Build and manage a structured sales funnel with clear stages and follow-up processes.
- Monitor lead volume, conversion rates, referral activity, and revenue performance.
- Identify gaps in the sale
