About the role: We are searching for a Case Manager I for our Casa Canal Permanent Supportive Housing Program This is an exciting role to gain Case Manager experience with the unhoused population. What you’ll do:

Intake and Assessment
- Screen prospective participants by verifying income and documentation, obtaining necessary consents, and reviewing eligibility with supervisors.
- Conduct comprehensive assessments (HMIS, VI-SPDAT, Housing Retention)



to evaluate participant strengths and risks while providing full orientation on program services, rights, and responsibilities.

Case Planning and Monitoring
- Conduct interviews and research to identify risks, stratify needs, and develop data-driven care plans.
- Establish participant-driven goals based on resource availability, consensus, and required level of care.
- Provide financial coaching—including budgeting, tax filing (EITC), and credit repair—to support long-term stability.
- Link participants to internal SHELTER, Inc. programs and external community resources, ensuring minor children are enrolled in school.
- Maintain a caseload of 20 households, facilitating case conferences and acting as a liaison between service partners and participants.
- Perform routine reassessments and proactively adjust care plans to improve outcomes in static or regressive situations.
