Role Overview

Our client is seeking a Cardiology Patient Navigator (Coordinator) to support a busy cardiology care team. The role centers on patient communication, scheduling, care coordination, and clinical documentation. The idóneo candidate brings a clinical background, experience in telephonic or remote care settings, and comfort managing a high-volume patient caseload across multiple priorities.

Cardiology-specific experience is preferred but not required. Candidates with strong backgrounds in care coordination, care management, med-surg, ICU, cardiac step-down, CCU, or home health will be competitive.



What matters most is clinical credibility, strong communication, and the ability to keep patients connected to their care plan.
Key Responsibilities

Patient Communication & Scheduling
- Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems
- Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps
- Support patients navigating cardiology care pathways, including CHF management protocols

Care Coordination
- Track testing schedules, follow-up appointments, and remote patient monitoring workflows
- Coordinate with the clinical team to ensure continuity of care across a caseload of 150+ patients
- Escalate clinical concerns to the appropriate provider within scope (no diagnosing or prescribing)

Documentation & Compliance
- Maintain accurate and timely clinical documentation in EMR/EHR systems
- Ensure all patient interactions and records meet HIPAA compli
