We are looking for enthusiastic and compassionate graduates or early-career professionals to join our Support At Home team in a full-time, 12-month maximum term opportunity based at our Bella Vista office.

This role will cover the Greater Sydney area.

About the Role

This is an excellent opportunity for someone starting their career in aged care, community services, social work, allied health or health services.

You will receive training, support and development while working alongside experienced professionals to help coordinate services for older Australians living at home.

In this role, you will support clients, carers and families by assisting with care planning, service coordination and ongoing client engagement to ensure individuals receive the support they need to live safely and independently at home.

Key Responsibilities

- Support the onboarding of new clients and assist with care planning and service coordination
- Build positive relationships with clients, families, carers and service providers
- Assist in coordinating services that support clients’ goals, wellbeing and independence
- Participate in client reviews and help maintain accurate care documentation
- Work collaboratively with internal teams and community providers
- Maintain confidentiality and deliver services aligned with Care Connect values and quality standards
- Provide compassionate and person-centred support to a diverse client base

What You’ll Bring





We welcome applications from recent graduates and candidates looking to build a career in aged care and community services.

To be successful, you will have:

- Minimum Diploma qualification in Health, Allied Health, Social Work, Community Services or a related field
- Strong communication and interpersonal skills with a genuine passion for helping people
- Ability to build relationships with clients from diverse backgrounds
- Good organisational skills and the ability to manage competing priorities
- A willingness to learn about the aged care sector and Support At Home program
- Basic computer skills including Microsoft Office
- Current Australian driver’s licence and access to a reliable vehicle
- Full Australian working rights
